How to Evaluate Rehab Options in California

No single rehab program in California is right for everyone. Fit requires verifying the exact provider entity, the requested level of care, the location and setting, regulatory status, qualifications and staffing, safety capability, schedule, whether housing is separate from treatment, private-plan network and authorization rules, financial terms, current availability and an individual assessment.
What Synergy can verify
Synergy directly provides detox, PHP and sober living. Synergy does not directly provide IOP. IOP and any other service Synergy does not directly provide may be considered only through a consent-based referral to a separate provider, with no promise of current partner availability.
Synergy accepts private insurance only at this time. Medicare and Medicaid are not accepted. Acceptance depends on the specific private plan, requested service, eligibility, current availability and other requirements. No benefits review, authorization, coverage, payment, admission, availability, placement or outcome is guaranteed.

Private insurance checklist
Plan and provider decisions control the result. Confirm each item with the plan and with the provider rather than assuming a general rule applies.
- The exact private plan, including the plan name and identifiers shown by the plan itself.
- The specific provider entity and the specific service location being considered.
- Network status for that provider and that location under that plan.
- Whether prior authorization is required and who is responsible for requesting it.
- Which medical-necessity and continued-review rules the plan applies.
- Deductible, copay, coinsurance and the allowed amount used in the plan calculation.
- Exclusions that apply to the requested service or category.
- Whether any housing charge is treated separately or excluded entirely.
- Appeal and review rights if the plan denies a request.
- Current eligibility under the plan on the date care would begin.
- Whether the provider currently accepts that specific private plan for that specific service.

- What is the exact legal name of the organization that would deliver this service?
- What regulatory status applies to this service at this location, and how can it be checked?
- Which categories of staff deliver the service, and what credentials do they hold?
- What happens during a medical emergency, and what capability exists on site?
- What does a typical schedule involve, and who sets it?
- Which services are delivered by this organization and which would involve another provider?
- Is housing part of this arrangement, and is it separate from any clinical service?
- What are the costs, payment terms, deposits and refund rules?
- What privacy practices apply, and how should private health information be shared?
- How is family involvement handled, if at all?
- How is continuity or discharge planning handled when this level of care ends?
- What is currently available, and what would need to happen before a start date?

Checking regulatory status
California publishes information about substance-use facility licensing and program certification through the Department of Health Care Services, and FindTreatment.gov lists services nationally. Search using the exact legal name of the organization and the specific address of the service location, because names used in marketing may differ from the entity on record.
